Evaluation and treatment of chronic or slow-healing wounds, including wound assessment, dressing changes, and treatment
recommendations.
Chronic Disease Clinic
Short-term care for high blood pressure, diabetes, high cholesterol, and thyroid conditions, helping bridge the gap until patients can establish with a primary care provider.

Our Wound Care Clinic provides evaluation and treatment for chronic, difficult-to-heal, or recurring wounds. Care may include wound assessment, dressing changes, treatment recommendations, and ongoing follow-up when appropriate.

Chronic Disease Clinic
Our Chronic Disease Clinic provides short-term bridge care for common chronic conditions while patients work toward establishing care with a primary care provider. We help address immediate needs, review treatment plans, and reduce gaps in care during the transition. This clinic is not intended to replace ongoing primary care.

Specialty clinics are offered based on volunteer provider availability and may not be available every week. Appointments are required. Submit a specialty care request and our team will contact you regarding current availability and next steps, or call during clinic hours.
